Supports the Social Service Director in planning, developing, and directing the facility’s social service programs to ensure residents' medically-related emotional and social needs are met. Works within applicable federal, state, and local standards and facility policies to promote resident well-being and independence. Serves as a liaison with other departments and community agencies to support resident care and resources.
