Assists the Social Service Director in supporting the facility’s social services to ensure residents’ medically related social and emotional needs are identified and addressed in accordance with applicable regulations and facility policies. Works within a long-term care setting to support resident well‑being, coordinate with clinical teams and community resources, and maintain confidentiality and regulatory compliance.
