About this role
Assist in planning, developing, organizing, implementing, evaluating, and directing the facility’s social service programs to ensure residents’ medically related emotional and social needs are met. Support coordination with administration, medical and nursing staff, community agencies, residents, and families while maintaining compliance with federal, state, and local standards. Provide resident advocacy, information on financial assistance programs, and documentation of social service progress.
