The Social Services Director leads the psychosocial and discharge planning program for a 142‑bed skilled nursing facility, overseeing resident and family support, advocacy, compliance, and team development. Reporting to the Administrator, this role ensures compassionate, individualized care, smooth transitions, and regulatory readiness while guiding a small social services team.
