Provides medically related social work services in a healthcare or long-term care facility to support residents’ physical, mental, and psychosocial wellbeing. Acts as a liaison with residents, families, clinical teams, and community agencies to facilitate access to social supports and care transitions. Focuses on assessment, coordination, and advocacy to help residents achieve the highest practicable level of independence and quality of life.

Qualifications
