Social Work Case Manager

This role involves intervening with patients and families facing psychosocial needs, providing support in coping with chronic, acute, or terminal illnesses. The position requires coordinating discharge planning, offering crisis intervention, and serving as a liaison between patients and community resources. The role also includes assessing and intervening in cases of abuse, neglect, and domestic violence across all patient groups from neonatal to adult.
