Social Worker (BSW) | Advanced Care Planning

This role coordinates and implements Advance Care Planning services across Avera ambulatory clinic settings for high-yield patient populations. The position facilitates discussions regarding patient goals, values, and preferences for future healthcare decisions while providing education, counseling, and care coordination to patients, families, and providers. The social worker serves as a subject matter expert to promote patient-centered care and support value-based care initiatives across the Avera continuum.
