Social Worker – Care Management

The Social Worker – Care Management provides coordination and continuity of care for complex patients from pre-admission through post-discharge, acting as a catalyst for psychosocial outcomes, patient advocacy, and efficient resource use. Reporting to the Care Management Manager, the role requires a Master’s in Social Work, a Washington Clinical Independent Social Worker Associate License, and at least three years of relevant experience.
