Social Worker Care Manager

The Social Worker Care Manager coordinates complex discharge planning, provides supportive counseling, and conducts psycho‑social assessments for patients across the continuum of care. Working with providers, RN care managers, and multidisciplinary teams, they develop and adjust discharge plans to meet patient needs, ensure regulatory compliance, and minimize readmissions.
