Social Worker Case Manager

Inova Fairfax Hospital is seeking a dedicated and experienced Social Worker Case Manager to join the Case Management Team. The role involves assessing patients' biopsychosocial needs, advocating for patients to resolve barriers to care, and providing counseling and interventions related to treatment and end-of-life decisions.
