About this role
Gundersen Health System (becoming Emplify Health) is hiring a full-time Hospice Social Worker to join their hospice team, providing patient- and family-centered support within the health system. The position is 32 scheduled weekly hours (64 hours bi-weekly) with primary day shifts Monday–Friday and every 5th weekend on-call. The role includes a comprehensive benefits package and pay starting at $25.40/hour.

Qualifications
