Social Worker I

The Social Worker I provides psychosocial assessment, counseling, care planning, and discharge support to patients within the Adult Schizophrenia Program and Clozapine Clinic at Johns Hopkins Bayview. This role involves collaborating with an interdisciplinary team to address psychosocial barriers, coordinate post-acute services, and implement interventions for optimal patient outcomes. The position requires conducting individual, family, and group psychotherapy while maintaining clinical documentation and obtaining necessary insurance authorizations.
