About this role
Assists the Social Service Director in planning, developing, implementing, and evaluating the facility’s social service programs to ensure residents’ medically related emotional and social needs are met in accordance with federal, state, and local standards and facility policies. Works with administration, medical and nursing staff, and community agencies to coordinate care, support discharge planning, and provide resident and family education on benefits and resources.
