Social Worker (LMSW) Emergency Room

The Social Worker II at Houston Methodist provides comprehensive clinical social work, psychosocial assessments, and complex discharge planning to patients in the Emergency Room and targeted units. This role involves collaborating with interprofessional health care teams to facilitate quality care, addressing social determinants of health, and leading hospital-wide psychosocial initiatives. The position also serves as a service-line leader for population disease management and assists in resolving financial and psychological barriers to care.
