Social Worker (MSW)

DaVita is seeking a Licensed Social Worker to provide compassionate support to patients and families managing End Stage Renal Disease across its Sioux Falls and Moccasin Creek dialysis clinics. In this full-time, part-day role, you will manage a caseload of 60 patients while helping them navigate the physical, emotional, and financial challenges of chronic kidney failure within an intimate outpatient setting. The position emphasizes advocacy, relationship building, and guiding patients through their rights and responsibilities in the dialysis community.
