Social Worker (MSW)

The Social Worker (MSW) provides quality psychosocial care to patients and coordinates care with the medical team while consistently applying the social work code of ethics. The role involves conducting thorough assessments of patient needs, developing psychosocial intervention plans, and facilitating access to community resources. The position also includes advocating for patients, coordinating transitions of care, and supporting discharge planning to decrease hospital readmissions.
