About this role
The Social Worker will join the Primary Care team in Wellesley, MA to provide psychosocial support to ambulatory patients and families, integrating medical, social, and resource needs into patient-centered care. This is a full-time, 40-hour-per-week hybrid role (four onsite days, one remote) focused on improving healthcare accessibility and supporting care coordination across multidisciplinary teams.

- Class D Passenger Vehicle Driver's License
