Social Worker

The Social Worker provides psycho‑social evaluation, crisis and trauma counseling, resource referral, and case management for patients and families in an outpatient addiction medicine setting. Working under the Director of Care Management and with clinical supervision, the role monitors psychosocial status, facilitates communication and decision‑making, and contributes to discharge planning as part of a multidisciplinary team.
