Social Worker

The Social Worker will join Vancouver Coastal Health's Primary Care Networks team in Powell River, providing bio‑psycho‑social‑spiritual assessments, short‑term counselling, and care coordination for patients and caregivers facing health and social challenges. Working within an interdisciplinary team, the role supports continuity of care, transitions, and navigation of services in alignment with BC social work standards and VCH values.
