Community of Care
The Social Worker will conduct in‑person assessments and re‑assessments for senior participants, develop and coordinate comprehensive care plans with an interdisciplinary team, and provide ongoing counseling and education to participants and families on aging, health, and PACE services. This full‑time role supports the mission of Senior Community Care of Michigan PACE by delivering person‑centered care to elderly individuals.
Community of Care is a nonprofit organization dedicated to helping older adults in rural Cass County, North Dakota remain healthy, connected, and independent in their homes. They provide free services to adults age 65 and older, including resource navigation, transportation to medical appointments, Faith Community Nursing visits, and Medicare Part D plan reviews. Community of Care also engages volunteers and community partners to support older adults and the families who care for them.
