Social Worker

This role provides counseling, crisis intervention, and transition planning services to patients and their families, including psychosocial assessments and implementation of identified services. The position involves collaborating with the interdisciplinary team to address complex care situations, identify community resources, and facilitate safe transitions of care. The Social Worker evaluates patient progress, documents interventions, and participates in process improvement activities within the care management department.
