Social Worker

A qualified Social Worker joins the Admission Avoidance team at Norfolk and Norwich University Hospital to support adults presenting to the Emergency Department. The role involves working within a Rapid Assessment Frailty Team to reduce unnecessary hospital admissions through proactive, person-centred interventions and timely decision-making. This position focuses on delivering high-quality care and promoting independence through effective partnership working with health and social care professionals.
