Social Worker

This role provides patient and family advocacy, support, and discharge planning within the hospital setting. The position involves assessing patient needs, coordinating care transitions, and communicating with payors and post-acute care facilities to ensure timely and appropriate care. The Social Worker also offers psychosocial support, crisis intervention, and education to patients and families throughout their hospitalization.
