Social Worker

This role provides social work services to assess and enhance the patient's and family's ability to cope with physical, emotional, and environmental problems related to illness and discharge planning. The Social Worker collaborates with the multidisciplinary healthcare team to establish appropriate discharge plans and provides counseling to assist with coping with new diagnoses, chronic illness, and palliative or terminal care. The position also involves advocating on behalf of patients and families to ensure they obtain necessary services.
