Social Worker

This role provides therapeutic intervention and social work services to patients and their families to enhance comprehensive care in the hospital and ensure continuity of care in the community. The position involves assessing psychosocial situations, coordinating discharge planning, and collaborating with healthcare teams to address patient needs and social determinants of health. The Social Worker also advocates for patients, maintains confidentiality, and develops networks with community and governmental services.
