Social Worker

This role involves supporting adults with older people and physical disabilities to live safely and independently in their own homes and communities. The Social Worker will collaborate with partners, carers, and community organizations to deliver strengths-based, personalized outcomes that matter to service users. The position is part of the Older People and Physical Disabilities team within the Neighbourhoods division of Adult Social Care.
