Social Worker

This role provides counseling, crisis intervention, and transition planning services to patients and their families at AAMC. The position involves conducting psychosocial assessments, developing care plans, and collaborating with interdisciplinary teams to facilitate safe transitions of care. The Social Worker also identifies community resources and ensures accurate documentation of interventions and reports.
