Social Worker

The Social Worker provides evaluative, preventive, therapeutic, and personal care services to meet the psychosocial needs of patients and their families at MercyOne West Des Moines Medical Center. This full-time role involves conducting psychosocial assessments, developing individualized case plans, and coordinating discharge planning to help patients achieve optimal health. The position requires collaboration with the healthcare team and community resources to support patients across the continuum of care.
