Social Worker

The Social Worker provides support to the hospice interdisciplinary team by conducting comprehensive patient assessments, identifying care planning goals, and offering psychosocial and emotional support to patients and caregivers at the end of life. This role involves collaborating with the interdisciplinary team, providing education on funeral planning and advanced directives, and assisting with bereavement support. The position requires conducting visits in residential settings and participating in an on-call rotation for after-hours needs.
