Social Worker

This position provides psychological and social support to patients and families, integrating counseling services with overall patient care under the oversight of a Licensed Clinical Social Worker. The role involves conducting psychosocial assessments, developing treatment plans, and facilitating patient adjustment during treatment, hospitalization, or community transitions. The Social Worker also identifies community resources and may perform case management duties to address patient needs.
