Social Worker

The Social Worker conducts psychosocial assessments, provides therapeutic interventions, and assists with discharge planning for patients and families at the Lindner Center of Hope. This role involves collaborating with clinical teams to develop treatment plans, offering crisis intervention and brief therapy, and educating patients and families on adapting to illness and life situations. The position requires maintaining accurate documentation, participating in quality improvement efforts, and ensuring post-hospitalization needs are met.
