Social Worker

This entry-level Social Worker role provides psychosocial services to in-center and home dialysis patients, collaborating with the interdisciplinary team to promote positive adjustment, rehabilitation, and improved quality of life. The position involves conducting patient assessments, developing care plans, and offering supportive counseling to address the emotional and behavioral impacts of Chronic Kidney Disease. The Social Worker also facilitates patient education, manages quality improvement activities, and assists with insurance and financial coordination to ensure comprehensive patient care.
