Social Worker

The Social Worker is responsible for implementing standards of care for medical social work services and adhering to all conditions in the Service or Employment Agreement. This role involves assessing the psychosocial status of patients and families, providing counseling and crisis intervention, and educating the interdisciplinary group on cultural needs. The position also includes maintaining clinical records, assisting with advanced directives, and serving as a liaison between patients and community agencies.
