Social Worker

This role involves providing social work services to participants within the Center, including one-on-one and group counseling, to address social issues and create individualized care plans. The position requires conducting comprehensive assessments and accessing community resources to ensure continuity of care across the continuum. The Social Worker collaborates with an interdisciplinary team to support older adults in living active, independent lives through the Providence PACE program.
