Social Worker

A registered Social Worker in the Hampshire County Council Continuing Healthcare team works within a multidisciplinary environment to support individuals with their onward care journey following hospital discharge. The role involves undertaking assessments in line with the Care Act 2014, engaging with multidisciplinary teams for NHS CHC assessments, and collaborating with individuals and families to transition them to suitable care provision.
