Social Worker

This role involves engaging patients and families to identify psycho-social needs and coordinate optimal, safe plans of care. The position requires educating, counseling, and referring patients as indicated while collaborating with a multi-disciplinary team and community resources. The Social Worker supports the Information Sharing Core Element of PFCC to ensure comprehensive patient support.
