Social Worker

This per diem/on call inpatient position supports patients at Santa Rosa Hospital and, as needed, at Healdsburg and Petaluma Valley. The role involves providing psychosocial assessments, counseling, and emotional support to terminally ill patients and their families, as well as palliative care services. The Social Worker collaborates with a team to help patients and families explore options, make decisions, and solve problems.
