Social Worker

This role involves conducting strengths-based assessments in customers' homes and performing hospital social work duties as part of the Discharge Outreach Recovery Service. The position requires managing a complex caseload, collaborating with individuals, families, and external agencies to develop support plans that enhance wellbeing and achieve positive outcomes. The successful candidate will work from the Arun Civic Centre office and travel county-wide to support residents.
