About this role

Hanford Post Acute is seeking a Speech Language Pathologist to evaluate, plan, and implement therapy services for residents' communication and swallowing needs in a skilled nursing facility. The role involves direct patient treatment, documentation, supervision of assistants, and participation in interdisciplinary care teams.

Responsibilities

Evaluate patients within 24 hours of physician referral
Develop and implement personalized treatment plans
Provide direct therapy and document patient progress
Supervise SLP Assistants and Clinical Fellows
Participate in interdisciplinary meetings and care conferences

Qualifications
