Speech-Language Pathologist

The Speech-Language Pathologist evaluates, diagnoses, and treats speech, language, cognitive-communication, and swallowing disorders for frail older adults in the PACE program. This role involves developing individualized care plans, providing direct therapy, and collaborating with an interdisciplinary team to promote functional independence and safety. The position also includes conducting assessments, educating participants and caregivers, and maintaining accurate clinical documentation.
