Substance Abuse Counselor

This role provides direct alcohol and substance abuse counseling and treatment services to homeless clients, individuals, and families to address social and substance abuse problems. The position involves conducting assessments, developing individualized treatment plans, and utilizing motivational interviewing to promote client stability and recovery. The counselor collaborates with a multidisciplinary PCMH team to coordinate care, facilitate groups, and advocate for clients' treatment and entitlements.
