Substance Abuse Counselor

The Substance Abuse Counselor supports the integrated model of care by assisting the Care Team in connecting patients to essential resources and services to meet their short and long-term goals. This role involves conducting comprehensive drug and alcohol screenings, assessments, and evaluations to determine appropriate follow-up care. The position requires coordinating client services from assessment through discharge and facilitating individual, group, and family counseling sessions.
