Support/Clinical

This role supports the RN and Social Worker by managing post-acute service referrals and handling non-clinical tasks to ensure smooth patient transitions. The position involves coordinating discharge services, validating patient information, and communicating with patients, families, and the care team. It also includes processing electronic referrals, following up on authorizations, and escalating issues to the Director of Case Management as needed.
