Supports Caseworker

The Supports Caseworker provides person-centered case management, coaching, and coordination to individuals through Supported Collaborative Living and Independent Living Supports. This role assists individuals in maintaining independence in their homes and communities while ensuring that necessary supports, services, and documentation are properly implemented.
