Targeted Case Manager

The Targeted Case Manager provides case management services to individuals with severe mental illness, substance use disorders, and/or serious emotional disturbances. The role involves assessing client needs, linking them to community resources, coordinating care, and monitoring their progress. The position requires maintaining regular contact with clients and families, collaborating with staff and external providers, and ensuring access to appropriate services. The role also includes transporting clients to community settings and maintaining accurate documentation of services provided.
