Targeted Youth Support Worker

The Targeted Youth Support Worker provides direct, relationship-based support and targeted interventions to vulnerable young people aged 13-18 and their families to help them overcome challenges and build resilience. This role involves working collaboratively with multi-disciplinary teams, schools, and partner agencies to develop intervention plans, monitor progress, and evidence positive outcomes. The position requires flexibility to work across various community settings and occasional evenings to meet the needs of young people and families.
