Telephonic Outreach Specialist

This role involves performing telephonic outreach for recently discharged and high-risk patients to facilitate discharge coordination and identify alternate treatment programs. The specialist consults with physicians, providers, and members to assess, plan, and coordinate services that meet individual health needs while promoting quality and cost-effective outcomes. The position serves as a liaison to physicians and members, providing telephonic support across all locations.
