Telephonic RN Case Manager

Alignment Health is seeking a compassionate Registered Nurse to serve as a Telephonic RN Case Manager for Special Needs Plan members in a fully remote capacity. This role focuses on improving the lives of seniors and complex care patients by helping them navigate their care journeys and overcome barriers to better health. The position involves conducting comprehensive health assessments, creating individualized care plans, and coordinating care with internal and external partners. Candidates must hold an active, unrestricted RN license in California and possess at least two years of clinical nursing experience along with one year of case management experience.
