This role serves as a key liaison between healthcare facilities, providers, and care teams to facilitate timely patient transfers and ensure seamless coordination of care within the Wisconsin region. The position involves coordinating logistics and bed placements for patient transfers, acting as a customer-centric point of contact for challenging callers, and maintaining accurate documentation of all transfer activities. The ideal candidate will collaborate with medical staff to determine the best course of action for patient admission and placement while adhering to regulatory requirements.
