Transition Coordinator - Pediatrics RN IV

The Transition Coordinator - Pediatrics RN IV plays a critical role in ensuring the success of St.Amant's business and operations by developing and supporting systems of care collaboration, patient flow, and family and staff support between Health and Transition Services, HSC Shared Health Children's Hospital, and community-based care. This position reports to the Manager, Infection Prevention and Control and Clinical Collaborative Practice and works collaboratively with partnered programs to provide respectful customer service. The role involves on-site responsibilities at multiple locations to facilitate early identification, care planning, and discharge planning for children requiring specialized supports.
